These Preventive Service Guidelines are based on scientific research and have been approved by physicians in our health plan networks.  Please check your plan prior to receiving services.  We recommend you check with your health benefits plan to understand which of these services are covered.

Newborn to 24 Months   Children 2 to 6 Years   Children 7 to 12 Years   Children 13 to 19 Years   Women 20 to 64 Years   Men 20 to 64 Years

These guidelines describe basic preventative medical services and immunizations that are suggested for asymptomatic, low-risk persons and should not be used as a substitute for medical care. These guidelines are also available for download PDF 178kb.
