A provider record ID is a unique identifier corresponding to a billing or rendering National Provider Identifier and Tax Identification Number.
When record IDs aren’t used: If you haven’t filed a claim with us under your record ID with a date of service within 24 months, we may cancel your record ID. That means you might be terminated from networks associated with this record ID.
If you want to keep your record ID active: File claims under your record ID within 24-month periods.
To apply to reinstate a canceled record ID: Complete the provider onboarding form.
