The following terms and conditions ("Terms and Conditions") govern your use of the Online Service (the Online Service Site) portion of the Blue Cross and Blue Shield of Montana Web Site (BCBSMT.com) (the Site), and by registering for use of or by accessing, viewing, or using the material on the Online Service Site, you indicate that you understand and intend these Terms and Conditions to be the legal equivalent of a signed, binding written contract, and that you accept such Terms and Conditions and agree to be legally bound by them. If you do not agree with the Terms and Conditions, you are not granted permission to use the Online Service Site and should exit immediately.
The Privacy Statement and Terms and Conditions of Use of BCBSMT.com on the BCBSMT Web Site also apply to your use of this Online Service Site, and both are incorporated in these Terms and Conditions by reference, as if set forth fully herein.
This Online Service Site provides (or may at a future date be revised to provide) online capability for certain individuals, providers, groups, agents or others to obtain or submit information or documents regarding premium billing and payment, eligibility, applications, enrollment, groups, creditable coverage, benefits, prior authorization, claims, claim payments, and to ask and obtain answers to questions. To use the Online Service Site you must register for the particular category of user on the Online Service Site and be issued a user ID and password. You will be allowed access to only those areas and functions of the Online Services Site granted to the user category for which you register. Individuals who perform work on your behalf may be required to register separately or to provide additional verification, information, documentation or acknowledgments as a condition of access and use.
You understand and agree that Blue Cross and Blue Shield of Montana may require you to re-register or to submit additional verifying information, documentation or acknowledgements at any time as a condition of continued access to and use of the online Services Site, and that Blue Cross and Blue Shield of Montana may terminate your permission to access and use the online Services Site at any time.
If you are a member or subscriber, you understand and acknowledge that the Online Services Site allows you to access, view and print your sensitive protected health information and that are responsible to safeguard and maintain the confidentiality of your user ID and password so that other persons are not allowed access to this information without your knowledge or consent. If you access and use the Online Services Site as a member or subscriber, you represent and warrant to Blue Cross and Blue Shield of Montana that you are the same individual whose information is accessed or used, or that you are duly authorized by such person to access and use such information on behalf of such person for the purposes for which it is accessed and used.
If you are a provider, employer, agent, agency or business entity, your permission to access, use and view material on the Online Services Site is expressly conditioned upon your agreement and compliance with the following terms:
You will limit access to the Online Services Site to those individuals in your organization or business who need access to and use the Online Services Site to perform their jobs on your behalf. Any other or additional access must be requested through the Blue Cross and Blue Shield Marketing Department.
You and your employees or other individuals working on your behalf understand and agree that the information viewable and printable online will be treated in the same confidential manner that sensitive medical and personal information must generally be treated in other aspects of your office or business activities, and that the information will be used and disclosed only as permitted by law.
You and your employees or other individuals accessing information on the Online Services Site on your behalf will exercise due care to ensure that any person cannot inadvertently view workstations and computer screens other than the person authorized to access the information, and will log off or lock their workstations if they will be leaving them unattended, so as to assure that unauthorized individuals are not able to access or view the information.
You and your employees or other individuals accessing information on the Online Services Site on your behalf will not share their logon IDs or passwords and shall exercise due care to safeguard their logon ID and password to ensure no unauthorized persons may log on to the system.
You will immediately terminate access capabilities of employees or other persons no longer working on your behalf or whose job functions no longer require access.
You represent and warrant to Blue Cross and Blue Shield of Montana that you and each person accessing information on the Online Services Site on your behalf is authorized and entitled by law and by any necessary authorizations or consents to access and use the material and information on the Online Services Site for the purposes for which it is accessed and used.
