Blue Cross and Blue Shield of Montana would like to take this opportunity to remind Montana providers that claims submitted by the following provider types for Healthy Montana Kids (HMK) members require referring provider information to be present on the claim. Claims from the following provider types will be denied unless the claims include referring provider information.
- Ambulatory Surgery Centers
- Audiologists
- Durable Medical Equipment
- Home Infusion
- Imaging Centers
- Laboratory
- Occupational Therapist
- Physical Therapist
- Sleep Studies Center
- Speech Therapist
