Blue Cross & Blue Shield of Montana is implementing electronic submission for contract requests, adding new providers to existing groups and provider demographic updates. Effective January 01, 2022 Blue Cross & Blue Shield of Montana will no longer be accepting paper submissions.

Contracts can be requested by completing the Provider On-Boarding form under Step 1 at the link below:

Provider Demographic Updates can be completed by using the Demographic Change Form at the link below:
