Mid-Market RFP Checklist

Develop a Quote

Email a completed Mid-Market RFP form to: TexasRFP51100@bcbstx.com

Use the "Quote a Group" eSales tool in your BAP account. The quoting tool allows you to create quotes for our fully- and self-funded small group plans.

Get New Sales Forms

Go to Form Finder. Click on Mid-Market Group from the left menu. You'll need to complete the following:

Benefit Program Application (BPA)
Employer application for group coverage.

Employer Group Information (EGI)
Submit required documentation on group size, MSP and COBRA status.

HMO Disclosure — Optional
Included within the new group BPA — only complete this section if you've sold HMO plans.

 

Submit an Enrollment Request

For member enrollment, use Smart Census OR the Group Enrollment Application/Change Form.

Smart Census Import Tool
Upload member enrollment, submit premiums, respond to requests and more.

After logging in to your BAP account, click Group>Enroll a Group>Mid-Market Group Enrollment. Instructions are on the e-Sales tools page in BAP.
