Quoting Tool
Use the "Quote a Group" eSales tool in your Blue Access for Producers account . The quoting tool allows you to create quotes for our fully- and self-funded small group plans.

Get New Sales Forms

Go to Form Finder. Click on Small Group from the left menu. You'll need to complete the following:

Benefit Program Application (BPA) for New Small Groups
Apply for new Small Group coverage.

Employer Group Information form (EGI)
Submit this form with the BPA for all new enrollments.

Group Enrollment Application/Change Form (Member Application)
Enroll new members. After implementation, this form can also be used to update memberships.

Montana Supplemental Employment Verification Form
Use with the UI5 or other proof of wages reports to document owners or other individuals who are employed but not listed on the group’s payroll records.

Enrollment Instructions

Get the steps to enroll a group. Through this process you can add employer information, plan information, enter census data (manually or via Smart Census Import Tool), submit billing details and more.

Smart Census Import Tool (Microsoft Excel file)
You can import member data using the Smart Census Import Tool. This Microsoft Excel file is not available via Form Finder. It is only available when you log in to your Blue Access for Producers account and follow these steps:

1. Log in to your producer account.
2. Click on the Group icon from the left panel if you’re using a laptop or from the top panel if you’re using a smart phone or tablet
3. Click on "Training, Admin & Tools"
4. Select “Group Sales Training Materials”
5. Click on the “Smart Census Import Tool – FI"
6. Save or download the file to your computer or company network
7. Follow the instructions on Using the Smart Census Import Tool
